Heartwood Place is a residence that provides memory care for those with Alzheimer’s or other forms of dementia.  Memory care goes beyond meal prep and medication management. Meaningful interactions each day are what make a difference in the lives of those with memory loss. Our homey community offers individuals an opportunity to connect with peers, family and friends, and health care professionals with ease.

 

The intentionality of our space is something you can see, feel, and hear. Each member of our team know residents by name. The building itself was built with memory care in mind. The purposeful design means ample natural lighting, open community spaces and a circular layout (it reduces confusion). What’s more, our care staff update charts and connect with one another using mobile devices. Therefore, hallways have very little sound pollution which results in an especially calm environment. Nothing here was retrofitted.

Services in the monthly base rent for memory care residents include:

  • 3 meals each day
  • Snacks throughout the day
  • Some modified diets
  • Weekly housekeeping services
  • Daily assistance with bed making
  • Weekly linen services
  • All utilities (except telephone and cable TV services)
  • Planned activities and exercise programs
  • 24-hour staff for supervision and assistance
  • Regular monitoring for general health and well-being
  • Medication management
  • Apartment maintenance

We can assist in designing a care and service program to fit individual needs for living as independently as possible. Therefore, if the services included in the monthly base rent are not enough to keep your loved one living independently, additional personal support services can be added. The personal support services fee is based on the actual services provided and is determined following a personal assessment conducted by our professional team.
